Is this hapenning to both channels at the same time?
Do you hear the bypass relay clicking when it happens?
First thing to do would be to check the solder joints on the connectors CN1a, CN1b, CN2a, CN2b and on the threshold and makeup pots.

Your amp has no volume control?
If not, you can place 2 attenuators between the MC624 and the amps.

To check that the unit is working do the following:
1. 
- send a low level audio signal (preferably a constant sine wave) to the input,
- set the level of the MC624 at 0dB, note the audio volume.
- bypass the MC624 completely and check that you get the same audio level.
2.
- send a strong audio signal (preferably a constant sine wave) to the input,
- set the level of the MC624 at -63dB, note the audio volume.
- Lower the audio signal by -63dB at the source,
- increase the level of the MC624 to 0dB and check that you get the same audio level than before.

Check that the in/out offsets are set to 0.
If one input or output is offseted by +5dB the the max level will be reached at -5 on the display.

I can add +12db of gain on all i/o but I don't get any attenuation.
When you are at -63dB attenuation, it is not possible to add more attenuation. This is normal. The offsets are added to the main level but the main level cannot go above 0dB or below -63dB.

Also at -63 I have level louder than I would expect.
Do you have all the -1dB steps from 0 to -63?

Ceci est normal sur les positions 10, 20, 30, 40dB du switch. Par contre ça ne devrait pas être le cas sur les positions 50 et 60dB.
Vérifiez le potentiomètre P1 à l'ohmmètre : de 0 à 10k entre la pin centrale et chacune des pins de côté, en fonction de la rotation.

The XLR input works, right?
Make sure you inserted Q8 in the correct A holes.
Check that you have the V+ voltage on CN3 pin4 (counting down), against pin1.
Check if you can measure the DI AC voltage on CN3 pin3, against 0V.

Il se peut qu'il y ait un problème sur les connecteurs J1a ou J1b. Vérifiez les soudures.

Pas normal.

Check the OUT pot (in circuit, after removing U1):
- between the side pins : 2 kohms
- between the center pin and one side pin: 0 to 2k depending on the pot rotation.
Check the solder joints on P2, R16, C10, C11.

- Continuity between finger and V+pin
- Voltage on V+, after removing the components.

Remove the tube, remove U1 and U4, place the phase switch in the center (mute) position and check again V-.
If you are still not at -15V, look for a component that is getting too hot.

This must be checked in the diode mode, red probe on the V- est pin.

Check the solder joints on D13, L2, PTC2.
Check the resistance of L2 (less than 1ohm).
Check the resistance of PTC2 (around 1ohm).

Check the continuity between the V- test pin and the V- finger on the card connector pin 14 (second finger from the bottom).

Do you have +15V on the V+ test pin?
Check the diode D14. No need to didconnect it.

The V+ voltage doesn't seem to get to U4 pin2.
Check the solder joints on the vertical to main PCB connections and on D14.
Remove U4 an measure again the DC voltage on pin2. It should be +15V.

Can you report the DC voltages on the 8 pins of U4, against 0V (beware of short-circuits with the probe tip!).

The bottom pin of R55 is connected to the bottom pin of D8, next to it. Not to C33.
